If your child has no problems, issues, or diagnosis's that could be helped by horseback riding, please still fill out the form by checking NONE and/or any other boxes that you deem appropriate.

Here you will find all forms needed to be signed by you and/or your Medical Doctor, to allow your child with special needs to ride.  This is for riders with IEPs, in self contained classrooms, with physical, mental, emotional, or cognitive disabilities that would not allow them to ride independently.
